Table of Contents has been revised to reflect the changes listed below:

 

Compliance Guide has been revised to incorporate new Policies and Procedures and Forms and has been merged with the Pre-Survey checklist.

 

Pre-Survey checklist has been removed and incorporated into the Compliance Guide.

 

SECTION 1 ADMINISTRATION REVISION/ADDITIONS

 

Pharmacy Code of Ethics Protocol has been added to explain the use of the Pharmacy Code of Ethics Form.

 

Pharmacy Code of Ethics Form has been revised to show Pharmacy Name and Effective Date.

 

Standards of Professional Practice Training and Acknowledgment Receipt Form has been added to acknowledge training received by pharmacy personnel.

 

Conflict of Interest P/P has been added to the program.

 

Leadership Committee P/P has been added for Pharmacies that use a committee to appoint members to a leadership committee for operational decision-making.

 

Leadership Committee Appointment Form has been added to notify employees of the members added to the leadership committee.

 

Pharmacy Patient Bill of Rights and Responsibilities P/P has been added to explain the use of the document “Patient Bill of Rights and Responsibilities”

 

Contracting with other Healthcare Providers P/P has been added.  Contains pertinent information regarding contacting with other Healthcare Providers.

 

Reference Library P/P has been added to provide a list of the reference materials that Pharmacy personnel should have access to.

 

SECTION 2 FINANCIAL MANAGEMENT REVISION/ADDITIONS

 

Operating Budget P/P has been revised to incorporate the use of the Operating Budget Template Form.

 

Operating Budget Form has been added to give the Pharmacy a template for the Pharmacy’s operating budget.

 

Beneficiary Accounts Receivables P/P has been revised to reflect Statement of Account Balances information.

 

SECTION 3 HUMAN RESOURCE MANAGEMENT REVISION/ADDITIONS

 

License and Certification Verification P/P has been revised to incorporate the use of the “License and Certification Log Form”.

 

License and Certification Log Form has been added to track the required verification of professional licenses and certifications of pharmacy personnel.

 

Continuing Education P/P has been added to provide information on tracking the continuing education of pharmacy personnel.

 

Continuing Education Tracking Log Form has been added.

 

Personnel Files P/P has been revised to incorporate additional documents that need to be maintained in pharmacy personnel files and the separation of files into a 3 file system: personnel, medical, and payroll files.

 

Personnel File Checklist Form has been revised to reflect the additional documents that need to be maintained in pharmacy personnel files.

 

Cultural Awareness P/P has been added to provide information on diversity with pharmacy personnel and beneficiaries.

 

Job Descriptions P/P has been added to provide information on the development of job descriptions, revisions to and training/retraining of pharmacy personnel.

SECTION 4 CONSUMER SERVICES REVISION/ADDITIONS

 

Beneficiary Goals and Outcomes P/P has been revised by adding text to allow “other appropriate licensed health care professional” to complete the Goals and Outcomes Form.

Beneficiary Information Form has been added to gather pertinent information on beneficiaries.

 

Beneficiary Plan of Care P/P has been revised.  See “Beneficiary Goals and Outcomes” above.

Medicare Beneficiary Product and Service Charges P/P has been added to ensure a list is made available, upon request, and lists the products and services offered by the Pharmacy with the associated cost.

Pharmacy Standards of Beneficiary Service Form has been revised to remove emergency coverage information on original and replaced with contact information for complaints.

 

Beneficiary Records P/P has been revised to enhance the listing of documents that should be contained in Beneficiaries’ records, as appropriate.

 

DMEPOS Prescription Review P/P has been revised.  Moved to Section 8 and revised to incorporate the Beneficiary Information Form.

 

Eligibility Protocols P/P has been added to ensure prescriptions are reviewed to determine appropriateness of dispensing to Beneficiaries based on their clinical condition.

 

Reporting Suspected Beneficiary Abuse/Neglect P/P has been added to outline the procedure for reporting suspected abuse/neglect of beneficiary.

 

Recognizing Signs of Suspected Abuse/Neglect P/P has been added to educate pharmacy personnel on signs of suspected abuse/neglect.

 

Suspected Abuse/Neglect Incident Report Form has been added for documenting a suspected case of abuse/neglect.
